Going Home
Your physician will inform you and the nursing staff when you
may be discharged. We ask that you make arrangements to
check out by 2 p.m. Please check with the Business Office
before you are released to ensure that all the necessary
information has been obtained.

When it's time for you to leave the hospital ... A discharge planner is available to help you plan for follow-up care and to coordinate community resources to meet your needs. The discharge planner will usually visit you in the first 48 hours of your stay. There is no fee for this service. If you have special needs, please let us know and the discharge planner will try to help you and your family find solutions.
